As you may not know, USA Hockey provides players with certain achievement awards. Cottage Grove Hockey Association would like to ensure that each player earning an award actually receives it. The three types of awards are noted below (Only one per season, per player):

• Hat Trick - Patch for any player who scores 3 Goals in a single game
• Playmaker -  Patch for any player who records 3 Assists in a single game
• Zero Club -  Patch for any goalie who records a Shutout in a single game
  • Note: All games MUST be a single USA Hockey sanctioned game
  • Must be a game, not a scrimmage with a goal differential of < 10 Limit of ONE award each per player per season

Please include the player’s name, date of the USA Hockey sanctioned game, award earned, include a hyperlink to the scoresheet, & include the mailing address of the Team Manager or Head Coach that should receive the patches from USA Hockey.

CGHA submits USA Hockey Awards in batch requests and sends just one email with all recipients names each month.  Please Allow 10-14 days for processing by the District Registrar.
